A sample of job analysis questionnaire follows. It is an open-ended job analysis questionnaire furnished courtesy of John A. Dantico, Consulting Principal at A.S. Hansen, Inc., management consultants of Chicago, Illinois. It illustrates a type of job analysis questionnaire that is used to collect job data. Although the sample may serve as a model, it is important to adapt the questionnaire to meet the specific needs of your organization. Incorporate questions that identify the essential functions of your jobs. Add questions that will yield adequate information to evaluate jobs according to your organization's job evaluation plan.
